This is the part of pain medicine that is quietly responsible for most of the durable results. An injection creates a window; the programme done inside that window is what stops the pain returning to where it was.
What it is
A structured programme of graded loading and movement, designed around your current capacity and the diagnosis, and progressed on a schedule rather than by how you feel on the day.
Who it suits
Almost everyone. It runs alongside injections and medication rather than after them.
How the programme is built
- Baseline measures — walking distance, sitting tolerance, stairs, sleep
- A starting level set below your current limit so early sessions are achievable
- A weekly progression, written down, with a rule for what to do on a bad day
- A flare plan, agreed in advance
- Review at four to six weeks, with the plan changed if progress has stalled
What progress looks like
Function moves before pain does, and that is the expected order. Walking further, sleeping through, returning to work hours — those are the measures we track.
Honest limits
It only works if it is done, and doing it during a difficult period is genuinely hard. Pacing and psychological support exist for exactly that reason, and asking for them is part of the plan, not a failure of it.
